New Data Collection Forms

As a reminder, all Federal and State Homeless Programs’ data collection forms were changed on 10/1/2019 to reflect the FY 2020 HUD HMIS Data Standards changes. You can find the updated data collection forms on our Forms and Instructions page.

Discard previously printed copies? If you printed forms with a date before 10/1/2019 in the footer, we would recommend discarding them as the response options to the Universal Data Elements “Previous Living Situation” and “Destination” have changed slightly.
