New Data Collection Forms

All program’s data collection forms were changed last week to reflect the State changes to data elements. You can find those at https://hmismn.org/forms-and-instructions/.Discard previous printed copies? If you printed forms with date between 10/1/2016 and 6/27/2017 in footer, we would recommend continuing to use them as the only change was that one question was removed.Only print enough copies to make it through September as there will be Federal/HUD changes effective October 1, 2017 that will require a new set of data collection forms.
