System Change for HYA and HTHP Outcomes Assessments

On May 1, 2018, ICA made a change in HMIS that now requires an answer to every question in the MN: OEO HYA Outcomes and MN: DHS HTHP Outcomes assessments when completing the Interim Review for a client. “Yes” indicates the client accomplished the outcome. “No” means they are either not focused on that outcome or are making progress but the outcome has not yet been achieved. Your funder has also sent communication regarding this change, so please refer to that email for more information. The HMIS User Guides found on our Forms and Instructions page have been updated to reflect this change. If you have any questions about how this will impact your data entry in HMIS, please contact the Helpdesk at: mnhmis@icalliances.org.
