Self Sufficiency Outcomes Matrix Requirement Update

The Self-Sufficiency Outcomes Matrix is an assessment and outcomes measurement tool that documents progress toward self-sufficiency across seventeen domains.

As of October 15th, Emergency Shelter projects funded by DHS Housing Support are no longer require to collect and enter the Self-Sufficiency Outcomes Matrix into HMIS.

Below are the programs that continue to required collecting and entering the Self-Sufficiency Outcomes Matrix data into HMIS:

• DHS Housing Support (HS) - Housing and Supportive Services only

• DHS Long Term Homeless Supportive Service Funds (LTHSSF)
