Reminder for FHPAP Providers Affected by the FHPAP Provider Split (Effective 07/01/17)

If you haven’t already done so, please move your clients to the new FHPAP providers that were emailed to you, as soon as possible.  We have begun to close the old providers; this will be complete by September 15, 2017.For printed instructions on how to do this (which were included in the training video), please go to this link.
