A new COVID-19 related project may be required to or choose to track information in HMIS. If HMIS is required for your project, that information will be provided to you by your funder. To request a new HMIS provider, submit a Provider Request Form. If you have new funds to expand an existing project and are unsure if you need to report this information via a separate provider, email our Helpdesk and we will provide guidance on whether you should submit a Provider Request Form.
