New RHY Grant? Renewed RHY Grant? Grant Ended? Send Us an Email!
New RHY Grants have been awarded, and we don’t know your grant numbers, new programs, or programs ending unless you tell us!
If you have a new RHY Grant (or any grant) please fill out the Provider Request Form so that we can create a new provider.
If you have a new grant number for an existing provider, please send it. Your grant number gets included in your RHY uploads, so it is important that we keep those up-to-date in HMIS. You may not see it, but it is included.
If your grant has ended, please let us know and we will close your RHY provider. If you are continuing programing without a RHY grant, let us know that as well!
Programming changes? Were you serving a different number of youth, or did you switch from serving single youth to parenting youth? Send that too! We rely on information you give us, and we love knowing about your programs.
