HTHP and HYA Outcomes Assessments Now Available!

HTHP and HYA Housing and Shelter grantees, please read this important update regarding Outcomes Assessments! Your new Outcomes Assessments are available to use! The assessments for the two programs are now different from each other, so please visit our Forms and Instructions page to download the new data collection forms. Detailed and updated user guides will be released the week of 9/25/17, which will contain program specific data entry instructions, as well as report guidelines. Please look for additional communication from your funders regarding the changes to the Outcomes Assessment.
