Form Correction: OEO HYA Emergency Shelter Assessment

A Minnesota HMIS Service Point user brought to our attention that the OEO HYA Emergency Shelter Assessment data collection form available on our website was missing a question.  That question is: “Extent of homelessness by Minnesota’s definition on the day before program entry?”  It has been added to the assessment form, now re-uploaded to our website.We always appreciate when users call to our attention possible oversights such as this one.  We apologize for any inconvenience this may have caused.  Please contact the Helpdesk at MNHMIS@icalliances.org with any questions.
