TheMN Homeless Prevention Assessment Tool, M-PAT for short, will be live in HMIS forthe new biennium beginning July 1st, 2019. The M-PAT will berequired to be entered into HMIS by all FHPAP Prevention providers.
A webinarwill be held on Wednesday, June 19th at 10am on the new PreventionAssessment Tool in HMIS. This webinar will provide more information on the toolin HMIS, step-by-step instructions, and a demonstration of data entry.
