DHS-OEO HYA Annual Reporting Deadline is August 15th and New Follow-up Summary Report

The annual reporting deadline for DHS-OEO HYA Youth Supportive Housing and Emergency Shelter projects is August 15th, 2021 for the reporting period from 07/1/20 – 06/30/21. Reports should be run and submitted directly to DHS OEO.

Reports due for OEO HYA Youth Supportive Housing projects:

• MIN-01-SAG-030 - MN Core Homeless programs 

• SHP-32-SAG-046 - HYA Outcomes

• SHP-32-DQR-289 - HYA and THP Follow-up Summary Detail**

**Please note that this report replaces the SHP-32-SAG-025 - HYA YSH Follow-Up Summary Detail report. Simply use the updated report for the next reporting period (and be sure to cancel any scheduled reports of earlier versions), along with your regular HYA reports.

• MIN-01-SAG-030 - MN Core Homeless Programs 

• SHP-32-SAG-046 - HYA Outcomes
