DHS Healthy Transitions to Adulthood (HTA) Program Updates
As of July 1st, 2022 the Healthy Transitions to Adulthood (HTA) program has ended. Agencies who were entering HMIS data for their HTA grant will need to work with ICA to close their HMIS provider.
Here are the steps needed to close your HMIS provider:
Run and submit your final reports to DHS.
Ensure all of your clients have been existed from your project with a 6/30/2022 exit date.
Email the Help Desk mnhmis@icalliances.org letting us know that all clients are exited and the HMIS provider is ready to be closed.
